The Board of Trustees is compiling their Medical Emergency List for the upcoming winter season.

If you need to get out for a medical emergency such as dialysis, chemo or similar, please first advise the Township to make sure your street is plowed out.  Provide a doctor's note confirming your treatment and submit it with this form.  

Meticulous will be advised and your driveway will be placed on their priority list.  

Note: Scheduled doctor's appointments are not a medical emergency.  

Also, please be sure you have enough medication as well as food for 3 or 4 days whenever a snow storm in forecast.  There is usually enough time to take care of these things before the snow starts falling.  

Please fill out the information and either mail it or drop it off to the Pheasant Run office.  All requests for medical emergencies must be in writing and signed and have a doctor's note..
